This 1981 sci-fi novel, Deathhunter by Ian Watson, the old and sick of humanity are able to meet their deaths in peace through the mysterious House of Death. Jim Todhunter is a guide for one of these houses. So when Nathan Weinberger commits a murder on live television, it becomes Jim's job to lead Nathan to his death. But the true nature of death, perhaps, is not what it seems...
